Financing

Payment for services are due at the time services are rendered unless payment arrangements have been approved in advance by our staff.

We accept cash, checks, MasterCard, Visa, Discover, and American Express. We will be happy to help you process your insurance claim form for your reimbursement. Any such request must be accompanied by a completed insurance form at each visit.

Your insurance is a contract between you, your employer and insurance company. We are not a party to that contract.
Our fees are generally considered to fall within the acceptable range by most companies, and therefore are covered up to the maximum allowance determained by each carrier. This applies only to companies who pay a percentage (such as 50% or 80%) of the "UCR". "UCR" is defined as usual, customary and reasonable fees for this region
Not all services are a covered benefit in all contracts. Some companies arbitrarily select certain services they will not cover and you are liable for those services.

There will be a service charge for any returned checks and for any accounts sent to collection. Balances older than 30 days may be subject to additional collection fees and interest charges of 1.5% per month.
